TERMS AND CONDITIONS OF ENTRY:

Who can enter:

Prize Draw is open to entrants who are UK residents during the Prize Draw period and prize fulfilment period, aged 18 years or over, and not employees of the Promoter and ITV, their immediate families, and anyone else professionally connected with the Prize Draw and who complete the survey entitled Wellness is dead, but feeling good isn’t (the “Survey”) made available via https://b6qrf8mimvp.typeform.com/wellnessisdead, (the “Website”) between 9 June 2023 and 19 June 2023. When you complete the Survey you may opt to be entered into the Prize Draw by providing your email address.

By taking part in the Prize Draw entrants accept and agree to the following terms and conditions. Any entrant who does not agree with any of these terms and conditions should not take part in the Prize Draw.

The Promoter reserves the right in its sole discretion to ask for evidence to verify the age, identity and/or eligibility of an entrant at any time, and to use any channels and methods available to carry out checks of any details provided. The Promoter may withhold delivery of a prize until it has received such evidence from the winner and failure to provide such evidence in a timely manner may result in forfeiture of the prize.

Entries must not be sent in through agents, third parties or on behalf of another person. Such entries are void. Bulk entries are void and cannot be accepted.

Opening/closing date & time:

Prize Draw opens for entries online via Website on 9 June 2023

Prize Draw closes for entries online at 23:59 on 19 June 2023

Winner selection and contact

The draw will take place within one (1) week of the closing date and time for all entries.

Subject to eligibility and compliance with all applicable terms and conditions, the first entry drawn at random from all eligible entries will win the prize.

All entrants must be contactable after 08:00 on 20 June 2023 and on 26 June 2023.

The winners will be contacted as soon as practicable on or after the draw. Reasonable efforts will be made to contact the winner to the email address provided when entering the Prize Draw. The Promoter will attempt to contact the winner three times in a three-hour daytime period over a two-day day period, and will indicate in each communication the deadline by which the winner has to respond to the Promoter.

Please note that if the winner does not reply to the Promoter by the deadline set out in the Promoter’s emails to the winner, or if the winner fails to meet any of the eligibility requirements or is otherwise unable to comply with these terms and conditions, this may result in forfeiture of the prize and the Promoter reserves the right to disqualify that entrant (without further liability to that entrant) and offer the prize to the next eligible entrant and thereafter until a winner is found.

It is the entrant’s responsibility to ensure that they control access to any email address that they use to enter. The Promoter and/or its group companies shall not be responsible for any loss the entrant suffers as a result of the Promoter dealing in good faith with anyone other than the entrant who has access to any such number (or any device using any such number).

How to enter & cost of entry:

Entrants may enter online by going to the Website and following the instructions. The entry will only be valid if entrants have completed and submitted answers to the Survey questions. There is no charge for entry but internet service provider’s fees may apply when accessing the internet.

If entrants choose to enter the Prize Draw, personal information they have provided to the Promoter as part of the Prize Draw entries and confirmation that they have fulfilled all entry requirements, will be used by the Promoter to administer the Prize Draw and fulfil the prize.

Prize:

There is one prize which consists of a £250 Planet Woo marketplace gift card which can be used towards the purchase of any items available on the Woo marketplace via https://planetwoo.itv.com/shop. The gift card will expire 12 months from the date of issue.

The gift card will be made available by email to the winner within 28 days of winner confirmation.

No cash equivalent or alternative prize will be given at the request of a winner and the prize is non-transferable and non-exchangeable.

The Promoter reserves the right to change the prize or any part of the prize to an alternative of equal or greater value for any reason in its sole discretion including without limitation due to circumstances beyond its control or if any part of the prize becomes unavailable. If for any reason the winner chooses not to take up the prize or any part of the prize, the Promoter reserves the right to ask the winner to confirm this in writing and, whether or not such confirmation is received, the Promoter will be able to retain or dispose of the prize (or part of the prize) at its own discretion.

The Promoter may refuse to provide a prize, or seek its recovery, in the event of non-entitlement under these terms and conditions or an entrant's breach of these terms and conditions, fraud or dishonesty, including but not limited to the use of technology which enables an entrant to evade applicable charges when entering the promotion.

General

In the event of any fault, mistake, misunderstanding or dispute concerning the operation of any part of the prize draw service, networks or systems, the decision of the Promoter shall be final.

The Promoter and/or its group companies cannot promise that the services relating to the Prize Draw will be free from errors or omissions nor that they will be available uninterrupted and in a fully operating condition. These services may be suspended temporarily and without notice in the case of any problem with any network or line, system, server, software, or for any technical malfunction or failure, maintenance or repair or for reasons reasonably beyond the control of the Promoter. The Promoter does not accept responsibility where all or any part of the service relating to the Prize Draw is discontinued, modified or changed in any way.

The Promoter or its group companies do not accept responsibility for any fault, malfunction, damage, loss or disappointment suffered by any entrant howsoever arising from participating in the Prize Draw or from accepting any prize whether due to any error, omission by the Promoter, its group companies or its or their employees, agents or others or by any other cause.

The Promoter and/or its group companies are not responsible for any damage or loss caused to an entrant (i) which is not reasonably foreseeable to such entrant and to the Promoter and/or its group companies when such entrant entered the Prize Draw; and (ii) is reasonably foreseeable to an entrant and to the Promoter and/or its group companies on entry but is only indirectly related to such entrant entering the Prize Draw.

The Promoter and its group companies will not be liable to reimburse the cost or expenses incurred in making the Prize Draw entry.

The Promoter and its group companies do not accept responsibility in the event that: (i) any entry is lost, delayed or not properly received, registered or recorded; or (ii) for any incorrectly addressed or undelivered entries to the Prize Draw; or (iii) for any disruption, delay or misdirection of such entries.

The Promoter and its group companies reserve the right to amend any element of the Prize Draw for reasons beyond their reasonable control, including but not limited to the unavailability of Prize Draw entry platform before the closing date, or for strike, lock-out, labour dispute, illness, act of God, natural disaster, adverse weather conditions, war, riot, civil commotion, accident, epidemic or pandemic, malicious damage, fire, flood and/or storm, compliance with law or governmental order, rule, regulation or direction, breakdown of plant, machinery or transportation. The Promoter and/or its group companies are not responsible for any loss or damage caused to any entrant as a result of any of these occurrences.

Entrants agree to reimburse the Promoter and/or its group companies in respect of any damages suffered by the Promoter and/or its group companies or any losses by the Promoter and/or its group companies resulting from any claim made by a third party in each case in respect of any matter arising from the entrant’s use of the services relating to the Prize Draw in breach of these terms and conditions or from the entrant’s violation of any applicable law or regulation.
